The patient portal is where as a patient of OFIS, you can complete relevant documents and access information relating to your upcoming treatment and hospital admission. Relevant questionnaires and post-operative instructions are also accessible.

Before your visit, please fill out the medical questionnaire through our online form  to ensure we have all the information we need to provide the best care possible. If you have been referred for TMJ or facial pain or orthognathic (corrective jaw) surgery, please also fill in the relevant questionnaire below, prior to your consultation:
